Case Coordinator

Agency Description: The mission of Neighbors Link is to strengthen the whole community by actively enhancing the healthy integration of immigrants. Neighbors Link provides innovative and impact driven programming to immigrant families and works to empower clients to successfully navigate community resources and to foster client leadership.

Job Summary: Neighbors Link is looking for a full-time, compassionate, patient and flexible Case Coordinator who will work closely with immigrant clients and families to help stabilize their lives in times of crisis, and provide ongoing support as needed. The Case Coordinator will assist individuals and families in connecting with community-based resources to address critical needs. The Case Coordinator will provide support in navigating community resources related to medical, mental health, legal and other social services throughout Westchester County. The Case Coordinator will also work closely with our legal team, the Neighbors Link Community Law Practice (NLCLP), to support the attorneys and paralegals in providing case management support for clients and families that need assistance connecting with additional community resources to support their immigration legal case. The Case Coordinator will be able to work collaboratively with staff, families, volunteers and partner agencies in our three locations in Mount Kisco, Ossining and Yonkers. This position will report to the Family Center Director. This position requires to be on site in more than one of our locations. Neighbors Link follows all COVID safety guidelines provided by the CDC.

Responsibilities:

  • Reflect the values and philosophy of Neighbors Link by creating a caring and nurturing environment based on mutual respect.
  • Work with individuals and families with a culturally sensitive approach that allows room for creativity and an opportunity to “meet clients where they are.”
  • Manage an assigned client caseload and document detailed case notes to ensure appropriate follow up and record keeping.
  • Support clients in crisis, assist individuals and families in accessing a wide variety of services, with a focus on empowering clients and cultivating individual autonomy once a crisis has been stabilized.
  • Choose an appropriate intervention technique that will encourage growth within clients and families in order to meet Neighbors Link overall mission of integration and empowerment.
  • Collaborate and strengthen relationships with community partners (i.e. Open Door, Northern Westchester Hospital, Mental Health Association, Catholic Charities, among others) in order to make direct referrals for clients with specific needs.
  • Conduct assessments and re-assessments of clients, including closing cases as appropriate.
  • Participate in team meetings, attend assigned trainings and educational workshops.
  • Consult with Family Center Director on new clients that may need immediate support based on needs/risk.
  • Establish service linkages, make necessary referrals and monitor service delivery to ensure that clients are being appropriately supported.
  • Provide regular updates to NLCLP and other departments on the status of clients they have referred for case coordination.
  • Assist with other program activities as assigned.

Requirements:

  • Fluent English-Spanish bilingual is required, bicultural preferred
  • Undergraduate Degree in Social Work, Case Work, Counseling, Psychology or related field
  • At least two years of previous experience in a similar position preferred
  • Adept at multi-tasking and ability to manage crises
  • Strong clerical/administrative skills including personal computer skills (i.e., Microsoft Office, Google Drive, Salesforce a plus)
  • Excellent communication (written and verbal) and organizational skills
  • Flexibility to work some weekends and/or evenings
  • Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community
  • Must be able to travel to all of Neighbors Link locations in Mount Kisco, Ossining and Yonkers
  • Ability to provide a supportive, culturally sensitive, and caring environment for all clients
